ABSTRACT

This chapter considers all the patient's symptoms and all the information available to doctors and then assesses the symptoms, signs and information given using this to choose the appropriate remedies for the problem, and extra information will be offered where available and appropriate. Discharge planning takes time, and should therefore commence at the time of admission. On admission, information is gathered so that referrals to other members of the multi-disciplinary team can be made appropriately. With the information to hand, have a discussion with the multi-disciplinary team to determine the best place for care to be offered. Explain the decision and the reasons for it to the patient and their relatives. Not all discharges are successful. Some patients and their families desperately want to try at home, and they should be supported in doing so.
