ABSTRACT

The Erie County Department of Senior Services began trying to coordinate long term care services in the late 1970s. Coordinated Care Management Corporation brought together the public and private sectors: from the outset its board comprised county officials and presidents of non-profit service agencies. Its primary mission was to devise and implement a system of services for the frail elderly; that mission has been accomplished. With increased county funding, supplemented with additional foundation and community grants, its activities have expanded to include provision of case management services directly. Communities and states devised various approaches to meeting the need for better long term care systems. The organizational models are the single-entity model; the single-entry/constituency-based client management model; and the multiple-entry/constituency-based client management model. There are many variations of each of these models and some existing programs may even incorporate elements of each model.
