ABSTRACT
On Lok Senior Health Services Community Care Organization for Dependent Adults (the CCODA) is a consolidated, community-based long term care program. Through the CCODA On Lok provides all health and health-related services needed by its frail elderly population from acute hospitalization to transportation, translation, home-delivered meals and recreation therapy. Thus the CCODA differs from both brokerage/channeling and traditional long term care approaches in that: the full range of social and medical services is integrated into a single health program; services are delivered by the same professionals planning them; and there exists an opportunity and implicit financial incentive to control costs with total control over all expenditures. The CCODA represents a third phase in the development of On Loks health program for the elderly of San Franciscos Chinatown-North Beach-Polk Gulch area. On Lok is one of the oldest community-based long term care programs.
