ABSTRACT

The eight demonstrations described in the monograph represent a subset of some of the better known long term care demonstrations which have been operating long enough to have some data but are recent enough so that key staff in the demonstration is still available. Before focusing on these projects to compare their characteristics and outcomes, it is important to acknowledge and describe, albeit briefly, some of the other community-based long term care demonstrations that could not be included in this monograph. Another interesting demonstration project is Channeling, a multi-state long term care demonstration derived from the case management service coordination pioneered by Triage. Fiscal integration refers to the extent to which the different funding sources involved in the reimbursement of long term care have been linked or integrated in a particular demonstration. Traditionally long term care has suffered from barriers resulting from multiple funding sources with different objectives.
