ABSTRACT

The following is an outline for a model of clinical intervention with the uncooperative patient based on a brief case study. The four step model is presented utilizing therapeutic interventions to illustrate each phase of treatment. The patient was a 68 year old male residing in a nursing home who had refused to leave his room for six months. He had been at the nursing home for 3 years following a two decade period of institutionalization in a state hospital. The Massachusetts Department of Mental Health was concerned about his increasing unwillingness to speak with nursing home staff, mental health professionals, and case managers as well as his progressive physical and emotional withdrawal. Subsequently I was requested to provide a clinical consultation.
