We appreciate the time and attention all three reviewers have devoted to our paper, and our ongoing research will profit from their suggestions. We limit our remarks here to Cook’s (chap. 6, this volume) comments, because they raise a number of points about issues that need further clarification and that can usefully be addressed here.
