ABSTRACT

A set of leadership themes emerges consistently across each of the preceding chapters. These themes suggest a set of effective leadership practices for addressing the social determinants of health and for advancing population health. These leadership practices developed from the cases and examples in each chapter are specifically targeted to supporting the reader in getting traction on change-now or in the future-in your location, in your department or organization, in your neighborhood or community, in the public policies that impact the health of those you serve now or will serve. As noted in the case of the Institute for Population Health Improvement, there is “purposeful” action to effect change. The cases provide examples of what “purposeful” looks like in practice, and how you operationalize intent.
