ABSTRACT
Plans are only good intentions unless they immediately degenerate into hard work. The return on investment is high in any industry where mishaps are prevented before they become mishaps. The purpose of incident reports is to help identify potential and actual risks and, thus, mitigate hazards. One of the goals is to determine whether an adverse event was preventable or unpreventable, and how preventive measures may be implemented. Incident reporting might increase if the reporting efforts are seen as important and productive. According to the Patient Safety Manual published by the American College of Surgeons, the objective of an incidence reporting system (IRS) is to identify 100% of adverse patient occurrences and potentially compensable events. The goal of constructive IRSs is to achieve a better understanding of a facility’s problems as they relate to patient safety.
