ABSTRACT

A 45-year-old well-dressed gentleman presents to your GP clinic. He says he works for the Government and that he has come to see you as he is worried about his lifestyle. He says his secretary recommended coming to see you as she reports he is drinking too much. He says he mainly drinks martinis and that this equals approximately 30 units per week. He describes several flings with women from abroad and that he is often getting into fights. He also is concerned he is getting too old for his job and is wondering if you can complete a sick note for him for a few days following a recent incident where he jumped off a moving train.
