ABSTRACT

A gentleman with an eyepatch and a long black beard is brought into the department. His name is Blackbeard. He has been helicoptered in from a ship off shore when he started complaining of acutely painful left leg. His leg suddenly became cold and painful and feels numb to his touch. He has previously had a similar problem in his right leg and now wears a wooden prosthetic ‘peg’. You examine the leg and find it to be pale and cold. You cannot feel any pulses in his foot; however, his radial pulse is fast and irregularly irregular. There is an ulcer present on the dorsum of the foot.
