ABSTRACT

JB, a 34-year-old man, is brought to the emergency department by a group of fishermen. He was found face down in the Mediterranean Sea and was unresponsive on being rescued from the water 4 hours ago. On arrival in the department he is combative and agitated. He has several passports with different identities on his person. On assessment he is maintaining his airway with no respiratory or circulatory disturbance. His observations are normal. He is able to obey your commands and he watches you when you speak; otherwise his eyes are closed. He is not sure where he is and keeps talking about a safe deposit box. He cannot remember what happened to him more than a few days before he was found. There is some bruising over his left temporal region and a small subcutaneous lump over his right shoulder.
