ABSTRACT

A 67-year-old woman presented to the emergency department complaining of difficulty in breathing. She had experienced increasing shortness of breath over the preceding weeks and this was exacerbated by lying down in bed at night. She also complained of worsening leg swelling over the previous fortnight. Her past medical history included hypertension and osteoarthritis. Her regular medications are listed below. She lived with her partner, smoked 10 cigarettes daily and drank approximately 20 units of alcohol per week.
