ABSTRACT

An 86-year-old woman was reviewed by her general practitioner following some recent changes to her medications. She felt well and had not experienced any chest pain, shortness of breath or falls/collapses over recent weeks. Her past medical history included atrial fibrillation, hypertension, ischaemic heart disease and heart failure. Her regular medications were: ramipril 5 mg OD, bisoprolol 5 mg OD, isosorbide mononitrate (modified release) 30 mg OD, and she had recently commenced digoxin, which had been uptitrated to 250 μg OD. She lived with her husband, smoked 10 cigarettes daily and drank approximately 18 units of alcohol per week.
