ABSTRACT

The stage has been set and invariably the next event is either a contact with the source of energy or a near-miss, depending on Luck Factor 1. In referring to the Cause, Effect, and Control of Accidental Loss model, it is proposed that Luck Factor 1 determines the outcome of an unsafe act, unsafe condition, or combination thereof. The contact with this source of energy is always as a result of an unsafe act or unsafe condition. In thousands of documented cases, unsafe acts have been committed and there has been no contact or no consequence whatsoever. The only plausible explanation is that Luck Factor 1 determines the outcome of unsafe acts and conditions. In investigating an accident in which a worker climbed a poorly supported electric utility pole and was killed when both he and the pole fell to the ground, it seemed a clear-cut case that the man had committed an unsafe act.
