ABSTRACT

This chapter introduces analysis process, Event and Contributory Factor Charts (ECFC) and its relation to a marine accident investigation reports according to the International Maritime Organization relevant resolutions and publications. The process starts chronologically from left to right, and in more complex accident cases, secondary and miscellaneous events can be added to the ECFC by inserting a line above or below the primary sequencing line. Safety studies involve examining a particular safety issue, like the effectiveness of the snap-back zone painted on a fore deck, accident trend from accumulated accident investigation reports, or other accident data. Recommended safety actions describe the safety issues and safety deficiencies and provide justification for the safety actions. Each recommended safety action includes a specific addressee in order to ensure the implementation of the actions. If safety actions were taken by the recipient, safety actions taken are excluded from the recommended safety actions, and written under a separate heading.
