No bibliographic references have been given in the chapters and Appendix A. In this appendix the main references for the different parts of the book are given, with annotations and a guide for further reading, with an accent on the literature sources that are the most useful for applied researchers. The publications are given in time order, and are not intended to be an exhaustive list.
