ABSTRACT

The Standard Drill represents one of my inventions: the multiplier effect. The multiplier effect flattens the lens and eyeball as much as possible in the first part of the drill before resorting to the indirect effect in the second part of the drill to flatten the eyeball some more. It is designed to treat a primary refractive error. The moderate myopic eye can take the drill’s high intensity due to the features of a primary refractive error. It is in the −0.75 D to −1.75 D range. The prescription for both eyes is the same or with a disparity of −0.25 D or less.

I also came up with a variation of the multiplier effect to deal with secondary refractive errors such as anisometropia and compound astigmatism: the indirect effect. It was like an invention within an invention. The indirect effect, or indirect influence as I sometimes called it, is a partial multiplier effect.

The multiplier effect can be thought of as a more intense indirect effect. Going by that definition, the Standard Drill is the most intense drill. The intensity of a drill is not positively correlated to the degree of improvement. A flexible ciliary muscle allows for a high intensity drill, but a ciliary muscle that is less flexible due to a partial spasticity requires a less intense drill. The type of refractive error also needs to be taken into account. The presence of astigmatism, for example, introduces another refractive error on top of myopia. The partial resistance requires another drill with a lower intensity factor. A decrease in intensity allows the motor cortex to manipulate a specific meridian of the crystalline lens instead of the whole lens.

Similar to the Preliminary Drill, the Standard Drill consists of the Distance Drill and the Snellen Chart Drill. The patient would perform the Distance Drill in the first part of the Standard Drill, and the Snellen Chart 142Drill in the second part, and then the Distance Drill again in the third part without wearing any ortho C lens. (Refer to the chapter What do Do for a description of how to perform the supplementary drills.)
