ABSTRACT

Introduction Provide a brief introduction that includes event details resulting in the need for an RCA, imminent activity in support of the RCA, etc. An RCA is about identifying connections between events and ultimately determining cause and effect. The RCA process separates cause and effect from other event relationships as shown in the list below. For the descriptions in the list, assume an event (A) has some relation to event (B); the following provide some insights regarding the potential relationship between (A) and (B):

Causal: The relation between cause and effect; a predictable result or flow from one action or event (A) to the next (B); an occurrence of (A) causes the occurrence of (B). Coincidence: The occurrence of events that chance to happen at the same time but seem to have some connection. Correlation: A relation existing between actions or events that occur together in such a way not attributable only to chance. (A) may not cause (B), but other events or circumstances may result in (A) and (B) occurring together. Contribution: The occurrence of one event (A) is not causal but contributory to even (B); event (A) is necessary but not sufficient to cause or predict event (B). Control: (A) has power or control over (B); (A) is not the cause of (B), but influences (B); given the absence of (A), (B) would still exist, but in a different state or behavior.
