ABSTRACT

In this chapter, the authors elaborate on the details of the logic and demonstrate that the use of a comprehensive cause-and-effect tool like the Logic Tree will identify causes that normally would not be picked up in a categorical Root Cause Analysis (RCA) approach. When properly using a cause-and-effect tool like the Logic Tree, it will drive us to identify the cause categories that played a role in the bad outcome, rather than our having to guess, when using a categorical RCA tool. The initial data for the purpose was collected in our 5 P’s effort in the categories of Parts, Position, People, Paper, and Paradigms. The 5 P’s data will ultimately be used to validate hypotheses on the Logic Tree.
