Topical clotrimazole preparations are indicated for the treatment of superficial fungal infections caused by dermatophytes or Candida albicans, and for treating pityriasis versicolor. Two patients showed sensitization to both clotrimazole and corticosteroids, as well as to the culprit antifungal creams. The number of reported reactions to clotrimazole was 13. Patch tests were positive to all topical medicaments containing the antifungal and to clotrimazole 1% pet., but not to metronidazole. Patch testing showed positive reactions to clotrimazole 5%, methylisothiazolinone, MCI/MI and the baby wipes she used on her anogenital skin before the start of the problem. The authors postulated without corroborating evidence that the maculopapular exanthema to oral fluconazole was the result of cross-sensitivity to primary clotrimazole sensitization. Although there are some unique and sporadic reports of simultaneous reactions, cross-reactions are very unlikely between clotrimazole and other imidazoles.
