ABSTRACT

Gallbladder malposition has been classified as medioposition or sinistroposition. A gall bladder in medioposition is displaced medially to the base of segment IV, but is located to the right of the ligamentum teres. In the case of sinistroposition or left-sided gallbladder, the gall bladder is placed to the left of the falciform ligament. There are three recognized varieties of left-sided gallbladder (LSGB): LSGB associated with situs inversus; true LSGB without situs inversus; and gallbladder located to the left of an anomalous right-sided ligamentum teres. Dissection in these situations requires a modified port position and proper orientation during dissection. ICG cholangiography may be helpful if the anatomy is unclear.
