ABSTRACT

This book came about because of the authors’ experience in teaching RCA in healthcare. Over the years, several training classes of various lengths were developed. The most recent evolution is an eight-hour training course. This book incorporates the curriculum for that course.

Who should receive training on RCA? Certainly, those who are tasked by healthcare organizations to facilitate RCA teams. These can include quality leaders, risk managers, patient safety professionals, caregiver safety coordinators, and clinical leaders. In addition, executives, directors, managers, and others in leadership benefit from training. Leaders need to understand how unintended event reviews are prioritized and when to employ the tool of RCA. Executives and Process Owners play key roles in thorough and credible RCA and should be trained in the Logic Tree methodology and action planning. Other professionals who may serve as Subject Matter Experts and RCA team members will be better prepared with advanced training. Training for these roles may not require the full course: customizing the training to the audience is encouraged.
