ABSTRACT

Patient assessment and diagnosis are very important steps in the patient care process; the management plan will not be appropriate. Without the appropriate assessment and diagnosis, treatment outcomes will not be achieved. Good prescribing requires good assessment, which has a good impact on the patient's health as well as the healthcare system. Patient assessment and diagnosis errors lead to an increase in admission rates to hospitals and healthcare facilities; an increase in morbidity and mortality; an increase in the cost of therapy, and a decrease in the quality of life and satisfaction with healthcare services. This chapter describes the patient assessment and diagnosis practice based on evidence-based practice, patient assessment errors, and patient diagnosis errors, moreover, the challenges of patient assessment and diagnosis in developing countries, and suggests recommendations to overcome them.
