ABSTRACT

Just as Chapter 1 discussed the basics of lubrication, in this chapter we delve into the basics of ‘Root Cause Analysis’ or RCA. The term ‘RCA’ means very different things to different people. So, to clarify our message and how we will interpret ‘RCA’, we hone in on a specific definition, along with a specific RCA approach called PROACT® (field proven for nearly 40 years). During this summary exploration we expand on the fact that true RCA is a system and not simply a task. We discuss the architecture of a logic tree, which is a graphical, event reconstruction tool that utilizes evidence to guide its direction to identify the root causes and contributing factors. This chapter lays the foundation for all the future chapters as we explore six (6) lubrication degradation mechanisms via the construction of a logic tree.
