ABSTRACT

This chapter pulls together the observations from my three case study chapters to propose a new model for delivery of advanced generalist practice. I start in primary care, and specifically in general practice. With commentators noting that ‘if primary care fails, then the NHS fails’, this is the space which most urgently needs reform. I outline new proposals for healthcare Policy: an extended vision of whole person healthcare, focused on revised outcomes of patient burden, societal expectations and workforce satisfaction. I describe a new model of Practice built round four elements: a new Front Door, revised Prioritisation, rebundled Delivery and embedded Learning Spaces. And champion new support for Wise People, including training in the 4Es and the creation of Learning Spaces (Wise Hubs) to embed and support the work into universal everyday practice.
