ABSTRACT

This chapter presents eight methods and two indexes to perform safety analysis of engineering systems. The methods are hazards and operability analysis (HAZOP), fault tree analysis (FTA), technic of operations review (TOR), failure modes and effect analysis (FMEA), job safety analysis (JSA), control charts, interface safety analysis (ISA), Markov method; and safety indexes, disabling injury frequency rate (DIFR) and disabling injury severity rate (DISR).

At appropriate places, the chapter contains examples along with their solutions, and there are ten problems, at the end of the chapter to test the reader’s comprehension in the area. Finally, it is added that the sources of most of the material presented in the chapter are given in its reference section. They will be useful to readers if they desire to delve more deeply into a specific area or topic.
