ABSTRACT
Every engineering design has its limitations and its breaking point. By recognizing all the possible mechanisms of failure, a robust engineering design can be developed to minimize the potential risk. Risk identification starts by describing the system structure and its interface to the operating environment. Artificial intelligence and its shocking failures are discussed.
