The failure to identify hazards, assess the risks, and institute the necessary controls causes accidents root causes to exist. This in turn creates the high-risk behavior and high-risk condition which, as Luck Factor 1 would have it, results either in a near-miss incident (no loss) or an exposure, impacts or contact with a source of energy (loss).

The degree of exposure, impact, or contact is what causes the loss. The outcome of an exchange of energy is determined by fortuity or Luck Factor 2. The outcome may be injury to employees, damage to property, business interruption, or a combination.

The results of an accidental exposure, impact, or contact with a source of energy are unpredictable and the outcomes are fortuitous.
