The exposure, impact, or contact that leads to an exchange of energy can result in business interruption, damage to machinery, property, materials, and vehicles, or lead to personal loss in the form of injury or occupational disease.

Injuries to employees are one consequent of accidents. More property damage accidents occur than injury accidents, and there are always more minor injuries experienced for every serious injury recorded. The difference between a serious injury-producing accident and one that results in minor injury is often fortuitous. The same accident could have caused only property damage or resulted in a near-miss incident.

No safety system or SMS can guarantee a reduction in the number of injuries due to the three luck factors. Measuring safety controls that help reduce the likelihood, and that drive the risk into the As Low as Is Reasonably Practical (ALARP) area, are far more pro-active and accurate measurements of safety.
