ABSTRACT

The best method to stop the accident sequence is to stabilize the three dominos representing the hazard identification and risk assessment process, the accident root causes, and the high-risk behavior and high-risk workplace conditions. This is done by designing, implementing, and maintaining a structured health and safety management system (SMS). The SMS stabilizes the first three dominos and prevents the accident domino sequence occurring.

A structured SMS should be implemented into each workplace to address the risks arising from the processes within that workplace. The SMS is a formalized approach to health and safety management through use of a framework that aids the identification and control of health and safety risks and is the only way to reduce accidental loss from occurring. An SMS stabilizes the weakness in the safety management process and reduces the possibility of accidental loss occurring. It ensures that hazards are recognized on an ongoing basis, that the risks are assessed, and that accident root causes are eliminated before they manifest. This will reduce the probability of an accidental loss occurring.
