ABSTRACT

If the hazards arising out of the business have not been identified, and their consequent risks have not been assessed and evaluated, they cannot be managed or controlled. This creates a lack of, or poor management control, in the form of a weak or flawed health and safety management system (SMS), which is represented by the second domino in the chain of events leading to accidental loss.

Inadequate, weak, failing, or nonexistent controls in the form of a structured SMS gives rise to the root causes of accidents. This leads to high-risk situations, which then lead to an exposure, impact, or exchange of energy which results in a loss.
