ABSTRACT

In this first chapter, we will begin by looking at some of the core concepts and themes maritime casualty and incident investigators encountered during the investigation process. It is important to understand the core concepts and themes of hazard and risk as they will have a direct bearing on how you plan, prepare, and carry out your investigation. They will also influence how you interpret the root causes of the incident when concluding the investigation. To that end, we will begin by examining organisational safety management. Moreover, the principles of hazard analysis and risk management are widely applied within many organisations to minimise the risk of incidents from occurring in the first place, and where the risk cannot be removed, to mitigate the impact of those risks.
