ABSTRACT

Timing is usually important when people are involved in incidents. It is also important in most safety and environmental incidents. Causal factor charting specifically addresses the timing of events. It also tries to incorporate some of the logic that we saw in the fault and 5-Whys trees. In other words, it tries to combine timing and logic into one technique. Causal factor charting establishes the relative timing of events and sets the time frame of interest for the incident.
