ABSTRACT

System safety is a conscientious effort to avoid accidents by eliminating or reducing hazards associated with all work systems. The most effective means to avoid accidents is by eliminating or reducing hazards and dangers during system planning or development. The analysis must ensure that our people have received training in the use of tools, machines, hazardous materials and the environment in which they are required to work. The science of human factors attempts to design equipment that can be operated easily and rapidly with a minimum of undue strain. System safety analysis must be performed in order to identify new hazards as a result of engineering changes and upgrading. An accident or injury is a tough instructor, the test is first and the lesson is last. If we do not learn from our mistakes, we are bound to suffer many repeat accidents. As technology advances, machines become increasingly complex and efficient, and better able to replace humans in production processes.
