ABSTRACT

It is a known fact that the majority of all accidents are caused by the unsafe acts of people. Unsafe conditions are also known as physical hazards and they exist in poorly organized or improperly planned situations; usually because of error or lack of knowledge on the part of those responsible. Unsafe acts are also known as human hazards, and they exist in the minds of supervisors and employees for many reasons. It should be noted that accidents are invariably preceded by unsafe acts and/or unsafe conditions. Thus, unsafe acts and/or unsafe conditions are essential to the existence or occurrence of an accident. Some unsafe conditions are: inadequate guards, fire and explosion, excessive noise hazards, radiation exposure and inadequate illumination. By definition, all unsafe acts have the potential for producing future accident injuries or losses, an unsafe act may be an act of commission or an act of omission. Serious health hazards and other major concerns must be corrected promptly.
