ABSTRACT

Since a qualified person must complete a permit before each entry, the permitting process provides a systematic method for verifying that all elements of the entry program are in effect. This chapter discusses some organizational variables which influence permit design, identifies specific core elements that must be included on all permits, offers some practical advice on designing effective permits, and describes special permits and procedures related to hot work. Organizational structure and management philosophy will generally dictate who is going to fill out the permit. Specific core elements that should be considered for all permits include a general description of the space, an explanation of the reason for entry, identification of potential hazards, identification of the measures taken to control those hazards such as isolation, lockout/tagout and ventilation, provision for atmospheric testing, identification of special protective, communications and rescue equipment and a brief description of the emergency plan.
