ABSTRACT

Abstract After a demonstration project that results from Chapter 4 and Initial State Innovation Model Deployments in a number of states, a set of patient-consumer-information-centric shared services and cloud-based capabilities can be supplied as shared services and through the patient advocacy disease foundations as part of the neighborhood and community national programs. Also individual patients who want to manage their own care with improved techniques can be brought on board and supported with bridging agents. Both neighborhoods-communities and bridging agents are discussed in more detail in Chapter 10.
