Recognizing, evaluating, and mitigating potential failures is a key aspect of Design for Six Sigma (DFSS). This enables teams to reduce risk within the product, processes, and services that are being designed. Failure modes and effects analysis (FMEA) is a structured method for identifying and mitigating potential failures. Poka yokes are mistake-proofing devices that prevent defects from occurring. Both topics will be discussed in this chapter.
