ABSTRACT
Remember that any writing in a patient’s chart becomes part of the permanent record. You should not erase or alter any part of the chart documentation. You should also try to keep the documentation as objective as possible because you cannot predict who will be reading the chart in the future. These potential chart readers may not understand the context in which the writer made his inputs. Therefore, the following are things that should be considered sacred to progress notes.
