ABSTRACT

This chapter presents a case of a 19-year-old health care assistant who attends the psychiatry outpatient clinic for her first follow-up since discharge. She had been admitted with the first episode of paranoid delusions, running commentary hallucinations and has been diagnosed with paranoid schizophrenia. However, she has gained nearly 6 kg in weight over the 3 months that she has been taking olanzapine. She is extremely concerned about this weight gain and is keen to discuss this during her appointment. She is dressed smartly. She is pleasant, articulate and expressive. She does not have any psychotic symptoms. Her mood is euthymic. She understands the implications of her diagnosis and is keen on adhering to the prescribed treatment. Her blood pressure is 128/78 mm Hg. Her waist measurement is 33 inches. There is no remarkable finding on systemic examination. Her blood investigations were reported as being normal at the point of admission.
