ABSTRACT

From time to time it may be necessary to correct an error or mistake in the records. In addition to amending the record in the appropriate way, it would also be prudent to write the reason for the error to make sure that the correct information is communicated. Where the records relate to the detention and treatment of those with mental health problems, correction of records may only be made in certain circumstances, which are set out in the Mental Health Act 1983. Errors that can be amended include: leaving blank any of the spaces on the form, failure to delete one or more alternatives, patient's forename and surname not agreeing in different places on documents, and neither of the doctors providing the medical recommendation. Health professionals working in mental health must make themselves familiar with the relevant legislation.
