ABSTRACT

Undetected human error creates a latent error condition that can contribute to a future undesirable outcome such as harm to people or equipment. Individual Latent Error Detection (I-LED) refers to the detection of workplace latent error conditions that occur post task completion through the recollection of past activity by the same individual who suffered the error. Chapter 1 introduces the I-LED phenomenon and how it can contribute to making a safety system safer through enhanced resilience. The chapter also states the aim and objectives of the book, followed by a summary of each chapter.
