ABSTRACT

The following questions may be used to encourage vigorous classroom discussion of technical human error (at the man-machine interface) and managerial human error (in man-people systems). The form of the questions may add to content knowledge and could provoke many different answers. The pathway to understanding, comprehension, and acceptance of human error and its ramifications is not as easy as it may seem, and group discussions about the questions could be great facilitators. The discussion questions also could be used for content reviews, self-learning, and self-evaluation. Just reading the discussion questions is something of an education; answering them may create a conversion to an advocate for error prevention. If there are appropriate answers, it suggests that the book may be used effectively as a future reference work or checklist reminder. While the examples used in the discussion questions are predominately real-life experiences, the described circumstances have been slightly altered and the names of people, brands, and companies have been omitted for privacy purposes. Some teachers may use the discussion questions for a Socratic method of instruction and leave the rest of the book for reference and checklist purposes.
