For all new prescriptions, their safety, effectiveness, and indications were assessed and patients were followed up at 3-month intervals. Appointments are always made for the first-time patient to the pharmacy. A paper-based system for documenting care was used. At that time, turnkey pharmaceutical care software programs did not exist and feedback was not provided to the patient’s physician or other caregivers. “You have to [remember] that this project was about defining the process of care. It was trial and error and even the pharmacists who participated did not always have a clear understanding of how we would do it,” says Wedin.
