In 1999, I published a review article on this subject,

in which I attempted to cover the literature up

to that date as completely as possible. For this reason, in this chapter, I will emphasize results that

have appeared more recently, referring the reader to the earlier review for a more complete

coverage of the subject. Thiemens has also recently published review articles.
