Exactly what is root cause analysis (RCA)? Is it simply another organizational buzzword that will end up being a fad, living out its half-life of six months and fading off into the sunset? We have all faced these “program-of-the-month” scenarios and find that it is difficult to implement any new thinking because of the way we have handled such new initiatives in the past. We are not only guilty of implementing such efforts ineffectively, but we are also victims to the “programs” that have been put upon us. With all of this said, we surely can understand the inherent distrust of the new corporate or facility initiative.
