Humans.are.an.important.element.of.the.health.care.system.and.they.are.subjected. to.various. types.of.errors..The.history.of. the.human.error. in.health.care.may.be. traced.back. to.1848,.when.an.error. in. the.administering.of.anesthetic. resulted. in. a.death.1,2.However,.it.was.only.in.the.late.1950s.and.the.early.1960s.when.serious. studies.concerning.medical.errors.were.conducted.3,4.These.studies.mainly.focused. on.anesthesia-related.deaths.
