To begin discussing the issue of root cause analysis (RCA), the foundation must be laid with some common understanding of key terminology. For instance, what are the key differences between problems and opportunities? Many people tend to use these terms interchangeably. The truth is, these terms are really at opposite ends of the spectrum in their definitions.
