ABSTRACT
Humans play a pivotal role during system/equipment design, production, operation, and maintenance phases. Although the degree of their role may vary from one phase to another, their interactions are subject to deterioration because of human error. Human error may simply be described as the failure to carry out a given task (or the performance of a forbidden action) that could result in disruption of scheduled operations or damage to equipment and property [1-3].
