ABSTRACT

The program must have a policy of encouraging all program personnel to come forward with concerns from their perspective, and all persons in program leadership positions have to avoid instant criticism of those who do come forward no matter how silly the concern turns out to be upon re‘ection. All concerns that come forward should be evaluated by the PIT and, if warranted, added to a list of active risks to be considered and possibly mitigated. Mitigation of risks means to reduce the probability of the risk having a negative effect on the program. Figure 25.1 illustrates a risk list example.
